Patient Participation Group (PPG)

We are currently looking for patients to join our PPG.

The PPG aims to have a selection of patients that represent the practice population who will meet at regular intervals together with a small number of practice staff to discuss ideas for improving services and facilities offered by the practice to our patients. It allows issues and challenges to be viewed from a patient perspective and helps ensure representative solutions are shared.

If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

If you prefer, you can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance

PPG support packs:
The CCGs' communications and engagement team has been working with the primary care team and Healthwatch Sefton colleagues to support practices with their PPGs.

Together, they have held a number of development events, attended by PPG and practice representatives and Healthwatch volunteers. At the last meeting in December 2021, there was a discussion on the development of the PPG support packs – one for practice use and one for patients.  Click here to open the patient support pack.
